Ontario-Wide Home Care

We started Aviora because too many families were being failed by the system - and we knew it didn’t have to be that way.

Inconsistent caregivers. Missed visits. Rural families told “we serve your area” and then quietly abandoned. We built a home care model that structurally prevents those failures - matched caregivers, real communication, rural coverage that holds. Aviora is currently accepting new clients and referrals across Ontario.

Our story

Why Aviora exists

The families we spoke with before building Aviora weren't angry about prices or package tiers. They were angry about broken promises. A caregiver who showed up once and was never scheduled again. A coordinator who stopped responding after week two. An agency that said yes, we serve your area - and then quietly couldn't staff it.

These aren't edge cases. This is how most home care in Ontario actually works. We built Aviora because the standard model is structurally incapable of delivering consistency. Not because agencies don't try - because the way they're built makes it impossible.

We don't just provide care. We coordinate it - from care planning to caregiver matching to ongoing adjustments - so nothing falls through. We match a primary caregiver to each client and build around their schedule. For clients who need more hours than one caregiver can cover, we build a small, consistent care team that is briefed on the same care plan. Rural logistics planned honestly. Families updated without asking. Every care plan walked through before anyone signs.

We're not the largest home care agency in Ontario. We're the one families stop searching after.

Understanding home care in Ontario

What families need to know before choosing a home care provider

What is private home care in Ontario?

Private home care in Ontario is non-government-funded personal support delivered in a client's own home. Unlike publicly funded care through Ontario Health atHome, private home care starts without a referral, has no waitlist, and gives families direct control over caregiver selection, scheduling, and the consistency of who enters their home. Aviora Healthcare is a private provider serving 120+ Ontario communities.

How does home care without a referral work?

With a private home care provider like Aviora, families skip the public system entirely. Step one: call or contact Aviora directly. Step two: a care coordinator assesses your situation and builds a structured care plan - no forms, no intake queue. Step three: a matched caregiver arrives within 24-48 hours. No physician sign-off. No OHIP. No waiting.

Private vs public home care in Ontario - what is the difference?

Public home care through Ontario Health atHome is funded by the government but comes with waitlists, rotating caregivers, and limited hours. Private home care like Aviora starts faster, guarantees caregiver consistency, and is tailored to each family's schedule and needs. Private care is not covered by OHIP but may be covered by private insurance or veterans' benefits.

What does a PSW do at home in Ontario?

A Personal Support Worker (PSW) assists with activities of daily living in the client's home - including bathing, dressing, grooming, mobility support, meal preparation, medication reminders, and companionship. PSWs trained through Ontario college-recognized programs are placed by Aviora after criminal record checks, reference verification, and credential confirmation. Every PSW is matched to each specific client's needs.

How we're different

This is what changes when a home care agency is built differently.

The founder is involved in coordination. This isn't a call centre operation. The people making decisions about your care plan are people who care about getting it right - not just filling a shift.

We vet our caregivers properly. Not every candidate who applies becomes an Aviora caregiver. We turn people away. Sending the wrong person into someone's home isn't a risk we're willing to take.

We respond fast because that's when it matters. Many families call after a hospital discharge or a sudden decline when care needs become more complex than one person can manage. Same-day response is standard. Families dealing with a care crisis don't have three business days to wait for a callback. We know that. We've heard what happens when agencies go silent.

We actually serve rural Ontario. A lot of agencies list rural communities on their website but can't staff them. We plan for travel windows and caregiver continuity from day one. Families outside the city aren't treated as an afterthought - they're part of the system.

Who built this

Rakshit Sharma - Founder and Director, Aviora Healthcare

Rakshit Sharma founded Aviora Healthcare Inc. after seeing the home care gap up close - not in a boardroom, but in the conversations families were having when no one else was listening. The pattern was consistent: rural families in places like Huron County, Renfrew, or Lambton who called four agencies and got four promises that evaporated the moment they needed a caregiver on a Tuesday morning. Families in the Kitchener–Waterloo–Cambridge area who were told care would start in 48 hours and waited three weeks. Seniors in Sudbury and Sault Ste. Marie whose families were told they were “outside the service area” mid-contract.

Rakshit built Aviora with one premise: the gap between what home care agencies promise and what they deliver is not a staffing problem - it’s a structural one. Agencies that prioritize volume over continuity will always rotate caregivers. Agencies that claim rural coverage without route-planning it will always fail rural clients. Aviora was designed around the constraints that break other agencies, not despite them.

He is based in Kitchener, Ontario, and is personally involved in the coordination of care plans, caregiver matching, and the ongoing quality of care across Ontario. Aviora’s guides, care resources, and client communications are reviewed through his direct operational experience in the Ontario home care sector, including familiarity with Ontario Health atHome pathways, employment standards governing caregiver scheduling, and the practical realities faced by PSWs working in both urban and rural communities.

Aviora Healthcare is currently accepting new clients and referrals from families, physicians, discharge planners, social workers, occupational therapists, and case managers across Ontario.

No surprises

What happens when you call Aviora for home care

Most families don't call because they don't know what to expect. Here is exactly what happens - no sales pitch, no pressure, no obligation.

01 - A real person answers

You speak directly with a care coordinator - not a call centre or an automated system. The call takes 20-30 minutes. You can ask anything.

02 - We listen before we suggest

We ask about the situation - what is happening now, what is needed, what has not been working. We give you an honest assessment, including if we are not the right fit.

03 - A care plan is built for your situation

If Aviora is the right fit, we build a structured care plan around your family's specific needs, schedule, and location. You review and approve everything before care begins.

04 - A caregiver is matched and briefed

We match a caregiver based on your care plan - not just who is available. The caregiver arrives briefed on your family's needs, routines, and preferences. Consistent. Reliable. Yours.

Our care team

Every caregiver is screened, verified, and matched

Every Personal Support Worker placed by Aviora Healthcare is thoroughly vetted before their first visit - and matched to your family's specific needs, not just dispatched based on availability.

Criminal record checks

All caregivers complete criminal record checks including vulnerable sector screening before their first placement with any client.

Reference verification

We verify references and work history for every candidate. Not every applicant becomes an Aviora caregiver - we screen carefully because getting it wrong isn't an option.

Credential confirmation

PSW staff hold certificates from Ontario college-recognized PSW programs. Credentials are confirmed before placement, not assumed.

Client-matched assignment

Our coordinators review each client's care plan, diagnosis, and preferences before assigning a caregiver - not just the next available person on the roster.

Standards & compliance

How we operate

Aviora Healthcare operates as a registered home care provider in the Province of Ontario, compliant with applicable standards under Ontario's home and community care guidelines. We support families across Ontario every day - from major cities to smaller communities where reliable care is harder to find.

  • PSW staff trained through Ontario college-recognized PSW programs
  • Vulnerable sector screening completed before first placement
  • Care plans confirmed with client and family before care begins
  • Operations compliant with Ontario home and community care standards

Our approach

Every care plan starts with a real conversation

We don't hand you a brochure and schedule a caregiver. Every client relationship begins with a thorough assessment - medical and daily living needs, individual routines, preferences, and what matters most to the family. We'll walk you through exactly what level of support is needed. The plan is structured around your situation, not a template.

Families always know who is coming to the home, when, and what they'll be doing. Our coordinators adjust schedules and respond to concerns the same day. If care isn't needed yet, we'll tell you. If something more structured is required, we'll guide you through it clearly. That's how a system works.

Values

What we hold ourselves to, every day

Consistency

One caregiver per client. Not dispatched randomly. Routines documented and honoured. This is the operating model - not a preference.

Dignity

Support that protects independence, privacy, and choice. Care delivered the way someone would want it in their own home - because that's where they are.

Communication

Same-day updates after visits. Proactive notice before anything changes. Families never have to chase us for information. That silence you experienced with the last agency? It ends here.

Reliability

Schedules that hold. Rural logistics planned honestly. If something genuinely can't work, we say so in the consultation - not after you've started and can't turn back.

Rural Ontario coverage

Rural Ontario coverage that's real - not a checkbox

Most home care agencies list rural regions in their service area. Many can't actually staff them. We've talked to too many families who were told they were covered and then spent weeks waiting for a caregiver that never came.

Aviora's rural coverage is built into the scheduling model from day one. We account for travel time, route planning, and caregiver continuity when we assess rural cases. If a community is outside what we can reliably serve, we'll tell you during the consultation - not after you've signed.

We serve communities across Northern, Eastern, and Southwestern Ontario, including areas outside Sudbury, Kingston, London, and beyond.

Straight answers

What families ask before choosing Aviora

Do I need a doctor's referral to get home care in Ontario?

No. Aviora Healthcare is a private home care provider. You contact us directly - no doctor's referral, no CCAC application, no Ontario Health atHome intake. Call (437) 446-7752 or book a free consultation online. Care starts within 24-48 hours across 120+ Ontario communities.

Will my parent have a consistent caregiver for each visit?

Yes. Aviora matches one primary caregiver to each client and protects that relationship as the operating model, not a goal. For clients with higher weekly hours, a small consistent care team is built so the people in the home are always familiar. Your primary caregiver learns your family member's routines, preferences, and needs. If any change is ever required, you are notified in advance - never surprised.

How quickly can home care actually start?

In most Ontario communities, Aviora can start care within 24-48 hours of your first call. We build the care plan, match a caregiver, and confirm the schedule during a single consultation. No waitlist. No intake queue. No system delays.

Does Aviora really serve rural Ontario - or just say it does?

Aviora builds travel time, route planning, and caregiver continuity into every rural care plan from day one. We serve 120+ communities across Ontario, including Northern, Eastern, and Southwestern regions. If we cannot reliably staff your area, we tell you during the consultation - not after you have signed.

How does Aviora screen and vet its caregivers?

Every Aviora caregiver completes criminal record checks including vulnerable sector screening, reference verification, and credential confirmation before their first placement. PSW staff hold certificates from Ontario college-recognized programs. Caregivers are matched to each client's specific needs, schedule, and personality - not just dispatched based on availability.

Need home care in the next 24-48 hours?

We guide families through this every day. No sales pitch. No pressure. Just a direct conversation about your situation and whether we're the right fit.
